The San Gabriel Pomona Valley Chapter of the American Red Cross is dedicated to providing resources and assistance to the community through our five lines of service: disaster preparedness, response, and recovery, health and safety training, blood drives, international services, and service to the armed forces & veterans. The Community Chapter serves 1.8 million people in 44 communities from La Canada Flintridge to Pomona and Duarte to Rowland Heights, covering 320 square miles.
We are a Federally Qualified Health Center (FQHC) that provides medical, dental, and mental health services to individuals across the San Gabriel and Pomona valleys. We practice patient-centered care and strive to serve each patient with the care that meets their individual needs. Foothill Family builds brighter futures by empowering children and families in our community to overcome challenges and achieve success in relationships, school and work.
Rooted in caring since 1926, Foothill Family provides a broad range of community-based mental health and social services to at-risk children and families in the San Gabriel and Pomona Valleys, Glendale, and Burbank.
For over 35 years, Inter Valley Health Plan has consistently served the Inland Empire with a level of caring and service unsurpassed by the competition. Arising out of the need for a strong community health plan in the Inland Empire, Pomona Valley Hospital Medical Center and a group of dedicated, private practice physicians joined to create Inter Valley Health Plan.
Landmark Medical Center has a mission of helping our residents reach their maximum level of potential and function. It is is hope and goal that the individuals we discharge will be well prepared to function in a less restrictive environment. We work with multidisciplinary teams, which include residents and their families. We also work in association with the assistance of a peer advocate who assists the facility and the county in the preparation of resident’s re-entry into the community. Our discharge planning process includes linkage with the step-down programs or case management agencies who will be working with the resident after discharge.

Medical services and pharmaceuticals are provided at no or low cost to low-income individuals and families based on their ability to pay. No one is ever turned away due to lack of funds. PCHC is a member of HealthCare LA IPA and the Inland Empire Health Plan – two large managed care networks serving Medi-Cal and Medicare patients. Additionally, services are provided to Los Angeles County residents through the My Health LA Plan and we actively enroll members into Covered California.
Unicare Community Health Center (UCHC) is a Federally Qualified Health Center (FQHC) with clinics located in Ontario, San Bernardino, Colton, Fontana, Pomona and East Los Angeles. UCHC is a grass root, community-based, 501(c)(3) nonprofit organization whose duty is to serve the unmet health care needs of low-income persons and their families.
