LANDMARK MEDICAL SERVICES

Landmark Medical Center has a mission of helping our residents reach their maximum level of potential and function. It is is hope and goal that the individuals we discharge will be well prepared to function in a less restrictive environment. We work with multidisciplinary teams, which include residents and their families. We also work in association with the assistance of a peer advocate who assists the facility and the county in the preparation of resident’s re-entry into the community. Our discharge planning process includes linkage with the step-down programs or case management agencies who will be working with the resident after discharge.
